Det varierer noe fra land til land hvor mye aktivitet som anbefales for barn med hjertefeil. I Norge er det sjelden det gis restriksjoner på trening. Den generelle anbefalingen er at barn og unge med medfødt hjertefeil bør delta i minst 60 minutter fysisk aktivitet daglig, som er på linje med andre barn. Varierte aktiviteter, som gir glede og mestring, er anbefalt3. Treningen bør alltid være lystbetont for barnet. For å sikre god helse på lang sikt, er det viktig å fremme fysisk aktivitet tidlig, og etablere gode aktivitetsmønstre som vil komme barnet til gode gjennom resten av livet.
Dersom foreldre er usikre på om deres barn eller ungdom bør delta i fysisk aktivitet, bør de ta opp dette temaet med hjertelegen. Legen vil da avklare om det er begrensninger og de kan diskutere hvilke muligheter barnet har til å være i aktivitet. For eksempel dersom barnet har en besvimelsestendens, er det lurt å unngå aktiviteter som kan skape farlige situasjoner dersom barnet besvimer, som svømming og alpint, men det er fortsatt aktiviteter som vil være trygge. For å få en indikasjon på barnets fysiske kapasitet er det også mulig å gjøre belastningstester som VO2max, og andre fysiske tester for å sjekke hvor mye hjertet tåler.
